This trade show-styled event will provide a forum to connect directly to public agencies and private businesses on issues and services that matter to you.

    One Care helps seniors and people with disabilities to live in their homes and communities with an array of supportive services. These services help people to maintain their quality of life and independence as they age at home. Programs include: home care, Meals on Wheels, transportation, exercise and wellness, adult day programs and many others.

    This revealing presentation will bring hearing health to the top of your mind!

    An overview of outpatient services offered through the South Huron hospital partnerships.
4:00 PM Grand Bend Community Health Centre Connected Rural Communities
    Learn about how the Grand Bend Community Health Centre is a connector and partner for people, families and communities to achieve health and well-being for all.
4:30 PM Alzheimer's Society Dementia Friendly Communities and Finding Your Way
    More people are diagnosed with dementia every year and choose to live at home in their community throughout their dementia journey. How can we better support people with dementia to live well? How can people living with dementia be leaders in building Dementia Friendly Communities? Let's talk about it.

    Learn about the supports and services available in our region to help individuals and families manage emotional well-being and mental health.

    Taking control of your health is an important part of living well with a chronic condition. Self management skills can be learned. Be an active member in managing your own health!

    Paul Ciufo, Certified Financial Planner (CFP), will explore the connection between financial security and overall physical, mental, and relationship health. Paul will outline strategies to obtain and maintain financial security for life.

    When help is needed most, this team will be there for you! Learn about a few of the programs and supports in place to serve the residents and visitors of Huron County.
7:00 PM South Huron Community Fund Investing in a Better Future for Community, Friends and Family
    Funds raised from the community create a permanent endowment that generates annual grants for local charitable organizations, including those in the health care field. The first series of grants are expected to be issued in the fall of 2018 by a committee of community volunteers.

    The presentation will provide an overview of the project, including how it all began, the information gathering process underway and the role of the Project Steering Committee.

    Get an overview of child & youth mental health services in Huron & Perth. HPC is the lead agency for Child and Youth Youth Mental Health and collaborates with all community partners.
