Death Registration


In the event of a death, a family member with the help of their funeral director completes the Statement of Death with information about the deceased. Once completed, the Statement of Death is submitted by the funeral director to the local municipal clerk’s office for registration. 

A death certificate is a document containing the details of a death that took place in Ontario. Once a death is registered you may apply for a death certificate online or by visiting the Service Ontario location at 388 Main Street South in Exeter.
